• Is under the age of 18

• Would have improved quality of life from treatment of occupational, physical or speech therapy

• Therapy is not eligible for coverage under insurance

• Equipment or resources not covered by insurance

• Is a resident of the Bartlesville, OK or surrounding areas (Pawhuska, Nowata, Dewey)

• Has financial need

 

Expenses that are eligible include:

• Services for occupational therapy, physical therapy and speech therapy. 

• Equipment or resources to aid in skill development or quality of life.
